Are you considering Homecare Services? Perhaps you’ve been told by a healthcare advisor that you or someone you care about cannot return home without help to assist you. Confused about what kind of help or how many hours you may need?

Heritage Homecare has been helping people stay at home and independent for 25 years.

1. Contact our HHC Care Manager for a personal interview. All it takes is a phone call. Your request for service will receive prompt attention. Our management staff will assess your needs and make recommendations to match your needs

2. Our Care management staff will prepare a plan of care which may include :

• Meal preparation, shopping, laundry, light housework
• Personal care bathing, continence care, dressing and ambulation
• Coordination of services with physicians, therapist, other providers etc.
  ♥ Peace of mind supportive care, companionship

3.Our staffing coordinator will select the most appropriate caregiver to fit your needs. You may request a change at any time by contacting your Care Manager. She will orient your caregiver(s) and supervise as needed. The Care Manager will follow- up with home visits to make sure your plan of care is functioning satisfactorily.
